唐僧、悟空和八戒上西天取经，被流沙河挡住去路。河里有个脖子上挂着一串骷髅头的妖怪，要吃唐僧。悟空一棒打去，妖怪吓得逃回水底，再不敢出来。

悟空让八戒下水去引他上岸来，那妖怪上了一次当之后，再不肯上来。

悟空去请观音菩萨帮忙。

菩萨早已劝服妖怪给取经人做徒弟，于是派木吒前去帮助悟空。

妖怪经木吒介绍，做了唐僧的第三个徒弟，唐僧给他取别名叫"沙和尚"。

木吒用观音给的红葫芦和沙和尚的骷髅项链，变出一条法船，送唐僧师徒渡过了流沙河。
